Print current grades and request transcripts and behavior reports (5th-12th) from your current school’s registrar and counselor/principal.  Then upload the documents in PDF format through FACTS.  Transcripts must include all current school grades up through the first semester of the most recent year (6-12). 

THINGS YOU WILL NEED:

• A PDF of transcripts (8-12) and current year’s grades
• A PDF of school behavior report (Grades 5-12)
• A PDF of IEP/504 and psycho-ed testing, if applicable
• A PDF of birth certificate
• A PDF of immunization records

In your application, you will be asked to provide the following references:  

• Current teacher (K-5)/Math or English teacher (6-12)
• Pastoral (preferred) or Character Reference (from anyone in authority who can speak to your child’s character; ie. soccer coach, piano teacher, youth leader, etc.)

THINGS YOU WILL NEED:

• Name and Email Address for Teacher reference
• Name and Email Address for Character reference

After a review of submitted documents, an interview will be scheduled with you and your student(s) to confirm Destiny Christian Academy can adequately serve your student.  Then upon acceptance, you will receive an invitation to enroll within 1-2 business days.

Submit the required enrollment forms online and pay the non-refundable enrollment fee.

K-12

• Early Enrollment $250 (enrollment must be COMPLETED by April 12)
• Regular Enrollment $500 (enrollment completed after April 12)
